Acceptable forms of payment are as follows...
Health Savings Accounts
Flex Spending Accounts
At High Caliber Chiropractic, we encourage the use of the HSA and/or FSA as the best form of payment. That is money set aside specifically for you to use on your health and improved wellness. It doesn't have to be used toward crisis care, but can be used toward preventative wellness visits. It's so important to stop the progression of an injury before it becomes worse. If we can be more preventative and proactive in your overall care, then we will be able to avoid major injuries that can take longer to recover from.
No Billing of Insurance
High Caliber Chiropractic does not bill a patient's insurance for services rendered. However, if a patient knows they have chiropractic benefits in accordance with their health insurance policy, Dr. Ryan provides a filled out insurance form to be submitted by the patient for reimbursement. In other words, the patients, those who have chiropractic benefits, can get reimbursed for services rendered and paid for on the back end. The insurance company will send a check right to your doorway. The billing form used is similar to an itemized receipt in that it does have the total amount paid by the patient for treatment provided by the doctor. The form is called a super bill and it includes all pertinent information about the patient, treatment and diagnosis so that the insurance company can process the reimbursement check to the patient in a timely fashion. All payment for services rendered are due at the time of treatment.
We do ask for 24 hours advanced notice if you are unable to keep your scheduled appointment. This allows us to be available for other patients in need of care. If the doctor arrives to your scheduled appointment time and the patient is not available, High Caliber Chiropractic reserves the right to charge the patient the full dollar amount as a missed appointment fee.
